Management of Urosepsis

Adults

  • Gentamicin (IV/IM) 120 mg every 24 hours for 5 days

    • Completion of 10 days of treatment should be guided by culture results

AND

  • Ceftriaxone + Sulbactam (FDC) (IV) 1 g every 24 hours for 5 days


Children

  • Gentamicin (IV/IM) 7.5 mg/kg every 24 hours for 5 days

    • Completion of 10 days of treatment should be guided by culture results

AND

  • Ceftriaxone + Sulbactam (FDC) (IV/IM) 80 mg/kg every 24 hours for 5 days


Alternative Regimens for Urosepsis


Adults

  • Gentamicin (IV/IM) 120 mg every 24 hours for 5 days

AND

  • Piperacillin + Tazobactam (FDC) (IV) 4.5 g every 6–8 hours for 5–7 days

OR

  • Meropenem (IV) 250–500 mg every 8 hours for 5–7 days


Children

  • Gentamicin (IV/IM) 7.5 mg/kg every 24 hours for 5 days

AND

  • Piperacillin + Tazobactam (FDC) (IV) as per body weight and specialist guidance

OR

  • Meropenem (IV) as per body weight and specialist guidance


Culture-Guided Therapy

Once urine and blood culture results are available, antibiotic treatment should be adjusted accordingly.
