Chronic Kidney Disease–Associated Metabolic Acidosis

Chronic Kidney Disease (CKD)-associated metabolic acidosis is a common complication of progressive kidney dysfunction resulting from the kidneys' inability to adequately excrete hydrogen ions and regenerate bicarbonate. It is typically diagnosed when the serum bicarbonate concentration is below 22 mmol/L. Persistent metabolic acidosis contributes to accelerated progression of CKD, muscle wasting, bone demineralization, chronic inflammation, protein-energy malnutrition, and increased mortality.


Early recognition and correction of metabolic acidosis are important components of comprehensive CKD management and may help slow disease progression and improve overall patient outcomes.


Epidemiology

Metabolic acidosis becomes increasingly common as kidney function declines, particularly in patients with CKD Stages 3–5. The prevalence rises with worsening estimated glomerular filtration rate (eGFR) and is highest among patients with advanced CKD and end-stage kidney disease. Persistent metabolic acidosis is associated with faster progression to kidney failure, increased hospitalization, and higher cardiovascular and all-cause mortality.


Risk factors

Common risk factors include:

  • Chronic Kidney Disease, particularly Stages 3–5

  • Declining estimated glomerular filtration rate (eGFR)

  • Diabetes mellitus

  • Tubulointerstitial kidney disease

  • High dietary acid load

  • Protein-energy malnutrition

  • Acute kidney injury superimposed on CKD

  • Poor adherence to CKD treatment


Pathophysiology

Healthy kidneys maintain acid-base homeostasis through excretion of hydrogen ions and regeneration of bicarbonate. As kidney function deteriorates, the ability to excrete daily acid load progressively declines, resulting in retention of hydrogen ions and depletion of serum bicarbonate.


Persistent metabolic acidosis stimulates muscle protein breakdown, increases bone buffering with subsequent bone mineral loss, promotes inflammation, and accelerates kidney fibrosis. These mechanisms contribute to progressive decline in kidney function and increased risk of adverse clinical outcomes.


Clinical presentation

Mild metabolic acidosis is often asymptomatic.

Patients with more severe disease may present with:

  • Generalized weakness

  • Fatigue

  • Muscle wasting

  • Reduced appetite

  • Nausea

  • Vomiting

  • Weight loss

  • Shortness of breath due to compensatory hyperventilation

  • Features of advanced chronic kidney disease


Clinical signs may include:

  • Tachypnoea or deep respirations

  • Muscle weakness

  • Features of malnutrition

  • Signs of advanced chronic kidney disease

  • Evidence of underlying CKD complications


Diagnostic criteria

CKD-associated metabolic acidosis is diagnosed in patients with chronic kidney disease who have:

  • Serum bicarbonate concentration below 22 mmol/L, usually confirmed on repeat measurement.

Diagnosis should be interpreted together with clinical findings and assessment of kidney function.


Investigations

Recommended investigations include:

  • Serum bicarbonate (venous or arterial where appropriate)

  • Serum electrolytes (sodium, potassium, chloride)

  • Serum creatinine

  • Blood urea nitrogen (BUN)

  • Estimated glomerular filtration rate (eGFR)

  • Arterial blood gas analysis when severe acidosis is suspected

  • Serum calcium and phosphate

  • Serum albumin

  • Full blood count

  • Urinalysis

Additional investigations should be guided by the suspected cause and severity of metabolic acidosis.


Management

Management aims to:

  • Correct metabolic acidosis.

  • Maintain serum bicarbonate within the normal range.

  • Slow progression of chronic kidney disease.

  • Improve nutritional status.

  • Prevent complications related to chronic acidosis.


Non-pharmacological treatment

General measures include:

  • Optimize management of the underlying chronic kidney disease.

  • Ensure adequate nutritional assessment and support.

  • Avoid nephrotoxic medications where possible.

  • Monitor kidney function and serum bicarbonate regularly.

  • Manage associated electrolyte abnormalities.

  • Refer patients with advanced CKD for nephrology assessment where appropriate.


Pharmacological treatment

Patients with serum bicarbonate below 22 mmol/L should receive oral alkali therapy.

Recommended treatment includes:

  • Sodium bicarbonate 650 mg orally every 12 hours. Adjust the dose gradually to maintain a serum bicarbonate concentration between 23 and 29 mmol/L.

OR

  • Calcium carbonate 500 mg orally every 12 hours, where appropriate.

Patients receiving alkali therapy should undergo regular monitoring of:

  • Serum bicarbonate.

  • Serum electrolytes.

  • Blood pressure.

  • Fluid status.

  • Kidney function.

Dose adjustment should be guided by the patient's response and laboratory results.


Referral

Refer patients to a nephrologist when:

  • Metabolic acidosis persists despite appropriate treatment.

  • CKD progresses to Stage 4 or Stage 5.

  • Severe or recurrent metabolic acidosis develops.

  • Dialysis is being considered.

  • Significant electrolyte disturbances or other CKD complications are present.


Complications

Potential complications include:

  • Accelerated progression of chronic kidney disease.

  • Muscle wasting.

  • Protein-energy malnutrition.

  • Bone demineralization and renal osteodystrophy.

  • Hyperkalaemia.

  • Reduced physical function.

  • Increased cardiovascular risk.

  • End-stage kidney disease.

  • Increased mortality.


Prognosis

The prognosis depends on the severity of kidney dysfunction, adequacy of correction of metabolic acidosis, and control of underlying CKD. Early diagnosis and maintenance of serum bicarbonate within the recommended range may slow CKD progression, improve nutritional status, preserve muscle function, and reduce the risk of adverse clinical outcomes.


Prevention

Preventive measures include:

  • Early diagnosis and treatment of chronic kidney disease.

  • Regular monitoring of serum bicarbonate in patients with CKD Stages 3–5.

  • Prompt correction of metabolic acidosis when serum bicarbonate falls below 22 mmol/L.

  • Optimal management of hypertension and diabetes mellitus.

  • Avoidance of nephrotoxic medications.

  • Regular follow-up to monitor kidney function and electrolyte balance.

  • Early referral of patients with progressive CKD to nephrology services.
