Laryngeal Cancer
Laryngeal cancer is a malignant tumor arising from the tissues of the larynx and is one of the most common cancers of the head and neck region. It occurs predominantly in men and is strongly associated with tobacco smoking and alcohol consumption.
The larynx is anatomically divided into three major regions:
Supraglottis – the region above the vocal cords.
Glottis – the region containing the vocal cords.
Subglottis – the region below the vocal cords.
The clinical presentation depends on the site of origin, tumor size, and extent of local invasion. Early diagnosis is essential because localized disease has better treatment outcomes.
Epidemiology
Laryngeal cancer is a common malignancy of the upper aerodigestive tract. It occurs more frequently in males and is strongly associated with lifestyle-related risk factors.
Glottic tumors often present earlier due to voice changes, while supraglottic and subglottic tumors may present later because symptoms develop after significant progression.
Risk factors
Risk factors associated with laryngeal cancer include:
Tobacco smoking.
Alcohol consumption.
Increasing age.
Male sex.
Occupational exposure to carcinogens.
Previous radiation exposure.
Human papillomavirus (HPV) infection.
Chronic irritation of the larynx.
Pathophysiology
Laryngeal cancer develops due to malignant transformation of epithelial cells lining the larynx, most commonly squamous epithelial cells.
Progressive genetic changes lead to:
Abnormal cellular proliferation.
Local tissue invasion.
Vocal cord impairment.
Extension into surrounding structures.
Regional lymph node metastasis.
Distant metastasis in advanced disease.
The pattern of spread depends on the anatomical site of the primary tumor.
Clinical presentation

Glottic tumors commonly present early with voice changes, whereas tumors in other regions may present with airway obstruction or swallowing difficulties.

Thyroid malignancy invading the larynx.
Hypopharyngeal cancer.
Diagnostic criteria
Laryngeal cancer should be suspected in patients presenting with:
Persistent hoarseness.
Stridor.
Progressive difficulty in breathing.
Neck mass.
Odynophagia.
Persistent cough.
Definitive diagnosis requires histopathological confirmation from biopsy of the primary tumor.

Liver function tests (LFT).
Imaging investigations
Chest X-ray (CXR).
Computed tomography (CT) scan of the head and neck.
Magnetic resonance imaging (MRI) of the head and neck.

Tumor extent.
Cartilage involvement.
Regional lymph node involvement.
Possible metastatic disease.
Endoscopic evaluation
Laryngoscopy.
Histopathological investigation
Definitive diagnosis is confirmed by:
Laryngoscopy-guided biopsy of the primary tumor.
